Tdap Vaccines for Rising 6th graders, 2012

Sixth grade students are required to have a Tdap vaccine before school begins in the fall. An easy way to get this vaccine is to attend a Tdap vaccine clinic at one of the following schools:
Wednesday, March 21 6:00 pm – 7:30 pm Enka Middle School
Thursday, March 22 6:00 pm – 7:30 pm North Windy Ridge
Thursday, March 29 6:00 pm - 7:30 pm Eblen Intermediate School
Thursday, April 19 6:00 pm - 7:30 pm Cane Creek Middle School
Tuesday, April 24 6:00 pm - 7:30 pm Asheville Middle School
Tuesday, May 22 6:00 pm - 7:30 pm Charles D. Owen Middle School
Tuesday, May 29 6:00 pm – 7:30 pm AC Reynolds Middle School
There is no out of pocket cost for parents for these vaccines!
A Vaccine Request Form is being sent home with 5th grade students. Forms will also be available on your school’s website or in your school office. Information should be brought to school no later than one week before the clinic date.
There will be a special “Express Service Line” for those who send their information back to school at least one week before the clinic date. This information includes:

Tdap Consent form-Moldovan
Tdap Consent form-Russian
Tdap Consent form-Spanish
ü Copy of child’s immunization record
ü Copy of your insurance card
Tdap vaccine clinics are sponsored by the Buncombe County Department of Health.
